Visits and follow-up

Bring your records and keep track of follow-up

Use a simple checklist to prepare for visits, record treatment and health changes, and note your next steps.

Organize your own records folder

Keep pathology and molecular test reports, images, discharge summaries and treatment records in date order. List your current medicines, supplements and any related discomfort. Bring previous records to follow-up visits so your doctor can understand the full course of your care.

Confirm who is responsible for the next steps

Before leaving, confirm your follow-up doctor, next appointment, tests to complete beforehand and how to get in touch if new problems arise. The frequency of follow-up and the tests needed vary with the condition, treatment and overall health. Follow the plan prepared by your care team.

Write down changes and concerns

Record new or persistent symptoms and changes in sleep, appetite, energy and mood. New symptoms do not necessarily mean that cancer has returned, but tell your doctor promptly. You can also discuss nutritional, rehabilitation or psychological support when needed.
