VISIT & FOLLOW-UP
Prepare for your next visit
One checklist. Three steps for your conversation.
Use your own notes to check each item. This site does not collect your responses.
Bring these records
- Previous imaging reports and any images your hospital asks you to bring
- Pathology, molecular testing and relevant laboratory reports
- Discharge summaries, operation notes and previous treatment records
- A list of current medicines, supplements and allergies
- Recent health changes, when they began and your key questions
Arrange records in date order and bring complete original reports where possible. Follow your hospital’s specific instructions.
Ask clear questions
- What do we know so far, and what is still uncertain?
- What will this test help us understand, and how should I prepare?
- How will I receive the report, and who will explain the results?
- What are the next steps, and what should I consider?
If a term is unfamiliar, ask your healthcare team to explain it in simpler language.
Confirm the next steps
- The date, location and team for your next appointment
- Tests to complete before your next visit and how to prepare
- Which health changes should prompt you to contact your healthcare team
- Official hospital contact details to use if a problem arises
Follow-up plans vary. Follow the plan agreed with your own care team.
